What is Home Health?
Home health care is a wide range of health care services that can be given in your home for an illness or injury. The goal of home health is to help you recover, regain your independence, become more self-sufficient, maintain your current condition or level of function, or help slow the decline.

How Do I Qualify for Home Health?
Your primary care physician or other health care provider will need to create a home health order referral to be sent to an agency of your choice or one that he or she works with regularly. Once an order is placed, then the home health agency will come to your home to talk to you about your needs and complete an initial evaluation and will work with your physician in creating a care plan to help meet your needs. The home health agency will continuously work with your physician throughout the duration of the service. Once the goals in the care plans have been reached, then the home health agency will perform a discharge.

Does My Insurance Cover Home Health?
Most insurances cover home health services. The home health agency will help you in determining your eligibility and will work with your insurance as needed.
